All Students are required to complete a PADI Diving Medical Questionnaire, available HERE. Depending on the responses to the questions asked, you may be required to complete a Full Dive Medical.

1. FROG DIVE requires you complete a full dive medical if you are you over 45 years of age and can answer YES to one or more of the following:

- currently smoke a pipe, cigars, or cigarettes;
- have a high cholesterol level;
- have a family history of heart attacks or strokes;
- are currently receiving medical care;
- high blood pressure;
- diabetes mellitus, even if controlled by diet alone;

2. You will be required to have a full dive medical if you have a pre-existing medical condition or history of medical conditions: i.e. Asthma or take preventative medications for chronic conditions (for examples, please refer to list below).

3. Full dive medicals are required to be completed at least 2 days prior to your course commencement date. If that is not possible, please contact the management at dive@frogdive.com.au.

4. If you are required to complete a full dive medical, the relevant form to be signed by the diving doctor can be found HERE.

These activities are all done in Confined Water (pool). All participants are required to complete the following form: PADI  Discover Scuba Diving Form (find it HERE) - for Introductory Scuba Dive Pool and Bubble Maker Waiver (find it HERE) - for Bubble Maker and Scuba for Kids.

1. The minimum age to participate the Bubble Maker/Scuba for Kids is 8 years old and maximum age of 12 years old;

2. The minimum age to participate in Introductory Scuba Dive Pool Only is 12 years old and there is no maximum age;

3. In case of pre-existing medical conditions (for i.e. see list below), a FROG DIVE representative will contact you to verify the possibility of the participant to do the activity, ensuring the safety of divers and staff at all times.

All participants are required to complete the form PADI Diving Medical Questionnaire, available HERE.

1. The minimum age to participate in this activity is 12 years old and there is no maximum age.

2. A full dive medical is compulsory for this activity if you have a pre-existing medical condition and/or are taking preventative medications (for examples, please refer to the list below).

3. If you are required to complete a full dive medical, the relevant form to be signed by the diving doctor can be found HERE.

When participating in pleasure dives/dive travel, all participants are required to advise FROG DIVE in case of any new medical conditions and/or any changes on pre-existing medical conditions stated when completing your Open Water Diver Course. If unsure, you can answer the PADI Diving Medical Questionnaire, available HERE, for guidance.

In this case, the diver will be required to present a letter from his/her doctor stating to be fit to dive. This letter must be signed by your doctor no later than 3 months prior to participating in the activity.

All participants are required to complete a PADI Medical Questionnaire that can be found HERE. In your responses, if you indicate that you are over 45 years old and have a chronic medical condition and/or are taking preventative medications, you will be required to present a full dive medical.

If your health has changed since completing your open water course and you now have a chronic medical condition and/or take medications to manage a medical condition, you will also be required to complete a full dive medical with a registered Dive Medical Doctor.

You will be required to provide a current dive medical, this medical must a no older than 12 months on the date of completion of your training programme. This medical must be through a Registered Dive Doctor.
